On Wednesday, December 31, 2025, the PCF PEC Survey website will be taken offline. As shared in this announcement, the PCF model ends for both Cohort 1 and Cohort 2 practices on that date.

The PCF Support line for practices will remain operations until Friday, February 6, 2026. Practices may contact PCF Support by email at PCF@telligen.com or by phone at 1-888-517-7753.
